
Cardiac Rehabilitation

Phase I
(Inpatient) Initiated while the patient is in the hospital immediately following a cardiac event or procedure. It focuses on gentle movement, basic education, and preparing for a safe discharge, with activities such as sitting up, deep breathing, and light walking.

Phase II
(Outpatient- Intensive cardiac rehab (ICR) Begins shortly after leaving the hospital or after a qualifying cardiac diagnosis; usually involves ~36 sessions of both monitored exercise and lifestyle education every visit.
This phase will begin with a stress test to identify your initial activity tolerance; then with your help, establish goals and a comprehensive individualized treatment plan to accomplish them.

Phase III
(Maintenance) Our phase III program is what makes us unique from other cardiac rehab programs. We provide physician prescribed individualized exercises programs using a series of specific instructional videos to be completed at home. We will also monitor patient engagement, patient progress and patient outcomes over time. This monitoring will allow us to triage indicators for further assessment and provide data analytics on daily functional activities for your referring provider.
