Who qualifies for intensive cardiac rehab?

A patient is eligible for Medicare‑covered Intensive Cardiac Rehabilitation (ICR) if they have at least one of the following:

  1. Acute Myocardial Infarction (MI) - Heart attack

    • Occurred within the preceding 12 months.

  2. Coronary Artery Bypass Graft (CABG) Surgery

  3. Current Stable Angina Pectoris

  4. Heart Valve Repair or Replacement

    • Surgical or transcatheter.

  5. Percutaneous Coronary Intervention (PCI)

    • Includes angioplasty and/or coronary stent placement

  6. Heart or Heart–Lung Transplant

  7. Medicare now covers CR/ICR for stable chronic heart failure (CHF) when ALL of the following are met:

    1. Left Ventricular Ejection Fraction (LVEF) ≤ 35%

    2. NYHA Class II–IV

    3. Being treated with medication 

    4. Clinically stable
